New Patient Information

Your first visit the doctor will want to focus on getting to know you, consult with you about any concerns or problems, give you a full dental examination, and create a treatment plan.

This first visit usually takes about one hour. If you need a lot of work done, our team of general dentists and specialists provide dental care for patients who need restorations, cosmetic dentistry, periodontal surgery, oral surgery and dental implants all in one office location.

Note: If you have decided to have general anesthesia for your dental procedures, be sure to read the instructions prior to receiving general anesthesia. You may also download this form and bring it with you to the office.

Insurance Accepted

If you have dental insurance we will determine which treatment costs are covered by your insurance plan, we will happily submit to your PPO dental plan, some of which we are in-network. If you have any questions about which insurance we accept, please call the office for more information.

Cash or Check

For patients without dental insurance; if you pay for your treatment in cash at least a week before treatment, we will offer you a 5% prepayment discount, certain conditions apply.

Debit & Credit Cards

We accept all major credit cards including Visa, MasterCard, Discover, and American Express. We also accept all debit cards.

Dental Insurance
